We are seeking an experienced Clinical Specialist to join our team at Sussex Community NHS Foundation Trust. As a key member of our Admission Prevention Team, you will play a vital role in supporting patients and staff in the emergency department.
Key Responsibilities
- Work closely with A&E staff to ensure seamless, safe, and effective discharge plans are in place for patients.
- Utilize your clinical expertise to support A&E staff in the patient pathway, facilitating rapid discharge and preventing hospital admission.
- Provide training and education to A&E staff on community services and discharge management.
- Maintain accurate records and data in line with information governance and data sets required for the service.
