About the Role

We are seeking an experienced and compassionate Care Coordinator to join our team in supporting individuals with learning disabilities and autism. This is a rewarding opportunity to make a positive impact in the lives of those we care for.

Key Responsibilities:

  • To work collaboratively with service users, families, and multidisciplinary teams to develop and implement person-centred care plans.
  • To ensure that all aspects of care are met, including physical, emotional, and social needs.
  • To maintain accurate and comprehensive records of service user information.
  • To communicate effectively with colleagues, managers, and external agencies to ensure seamless care provision.

Requirements:

This role requires a high level of empathy, understanding, and patience when working with individuals who may have complex needs. You must be able to demonstrate excellent communication skills, both verbally and written, and be able to work independently as well as part of a team.
