Job Title: Disability Support Coordinator

Salary: $55,000 - $65,000 per annum

About the Role

We are seeking a highly organized and communicative individual to join our Flexible Disability Support Unit as a Disability Support Coordinator. This role is responsible for coordinating the placement of care staff in clients' homes, managing rosters and schedules to ensure timely and accurate support, and liaising with recruitment services to support accurate recruitment.

Main Responsibilities
  • Coordinate placement of care staff in clients' homes to meet their specific needs.
  • Manage rosters and schedules to ensure timely and accurate support for clients.
  • Liaise with recruitment services to support accurate recruitment and manage absences and roster gaps promptly.
  • Maintain accurate documentation for reporting purposes and communicate effectively with clients regarding roster and care staff.
  • Work with clients to empower them in using their flexible supports budgets to improve life outcomes.
Requirements
  • A self-starter with initiative and drive.
  • A genuine willingness to advocate for and work with a diverse range of clients.
  • The ability to develop and maintain collaborative and trusting relationships.
  • Attention to detail and accuracy in data entry and resource management.
