About The Role

This part-time position (25 hours negotiable) is ideal for an experienced pharmacist looking for a new challenge. You will work closely with the prescribing team to optimize medication regimens, reduce polypharmacy, and promote adherence.

Key Responsibilities:

  • Lead on implementing changes in evidence-based prescribing across the patient population
  • Liaise with community pharmacists to support medicines adherence
  • Contribute to quality and safety improvements through practice-based audit and implementation of changes
  • Prescribe advice to prescribers in temporary non-availability of drugs
  • Ensure patient safety during transfers between care providers through reconciliation of prescribed medicines

Requirements

To succeed in this role, you will need:

  • Completion of an undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council
  • A minimum of 2 years postgraduate experience in pharmacy
  • Maintenance of self-development through continuous professional development activity
