Care and Support Navigator

About the Role
">The Leicestershire County Council is seeking a highly skilled Care and Support Navigator to join its team. The successful candidate will be responsible for providing comprehensive initial contact and duty functions, ensuring that queries, issues, and referrals are prioritised accordingly and progressed in line with service response timescales.
As a key member of the HART Service team, the incumbent will be the first point of contact for internal and external customers, providing culturally appropriate reablement support to meet identified goals and support needs. They will also programme the efficient deployment of resources to maximise utilisation and ensure timely discharge from acute and community hospitals.
The ideal candidate will have a minimum of 4 GCSEs (A-C grades) including English Language or equivalent qualifications. They should possess experience working in local government, particularly within Adult Social Care settings, and be able to manage risk and ensure the best outcome for customers.
A strong background in customer service, risk management, and teamwork is essential. The successful candidate will also have excellent written and verbal communication skills, be able to learn and process new information quickly, and demonstrate quick thinking and problem-solving abilities. Additionally, they should have access to a vehicle and hold a valid driver's licence.
This role requires a flexible approach to work, with a rolling rota shift pattern including weekends, early, late, and day shifts, as well as bank holidays. If you are a motivated individual with a passion for delivering exceptional care and support services, we encourage you to apply.
Job Requirements

Salary: £26,279 - £27,462 per annum
