This position involves delivering high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals. As a Primary Care Specialist, you will work collaboratively within a multidisciplinary team, providing a range of nursing interventions and supporting the holistic needs of patients with acute, chronic, or complex health conditions. The role requires strong clinical skills, excellent communication, and a passion for delivering patient-centred care in a community setting.

The key responsibilities of this role include:
  • Clinical Care
  • Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
  • Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
  • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
  • Provide health education and advice to patients and carers to support self-management and promote independence.
  • Community-Based Care
  • Work autonomously in patients homes and community settings, ensuring safe and effective care delivery.
  • Manage a daily caseload, prioritising tasks to ensure timely completion of nursing interventions.
