About the Role

We are seeking a skilled and dedicated Nurse Practitioner to join our team at Stockport NHS Foundation Trust. As a key member of our District Nursing Service, you will play a vital role in providing high-quality patient care and contributing to the delivery of our Trust's vision.

The successful candidate will have a strong clinical background, excellent assessment and organisational skills, and the ability to work effectively in a fast-paced environment. You will be responsible for providing care to patients in their homes, as well as in residential settings, and will work closely with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Provide high-quality patient care, including assessment, planning, implementation, and evaluation of care plans
  • Work collaboratively with other healthcare professionals to ensure comprehensive care delivery
  • Develop and maintain strong relationships with patients, families, and carers
  • Contribute to the development and implementation of care pathways and protocols
  • Participate in quality improvement initiatives and contribute to the continuous improvement of our services

Person Specification

We are looking for a candidate with:

  • A strong clinical background and relevant experience in District Nursing
  • Excellent assessment and organisational skills
  • The ability to work effectively in a fast-paced environment
  • Strong communication and interpersonal skills
  • A commitment to delivering high-quality patient care
