Job Summary

We are seeking a skilled and dedicated Community Health Worker to join our team at Devon County Council. As a Community Health Worker, you will play a vital role in supporting families with children aged 0-8 years, promoting their health and wellbeing, and helping them to reach their full potential.

Key Responsibilities

  • Work with families individually or in groups to provide support and guidance on child development, health promotion, and behaviour management.
  • Conduct 1 and 2 1/2 year development reviews to assess children's progress and identify areas for improvement.
  • Integrate with Health Visitors, School Nurses, and Staff Nurses to provide a comprehensive assessment of families' needs and develop targeted support plans.
  • Provide health promotion and development topics, such as behaviour management, toilet training, and sleep management, to support parents and carers.
  • Work collaboratively with other agencies and professionals to ensure a seamless service delivery.

Requirements

  • Passion for working with children and families.
  • Comprehensive understanding of children's safeguarding and the Healthy Child Programme.
  • Knowledge of the public health agenda and its relevance to early years development.
  • Ability to demonstrate the qualities outlined in the person specification and job description.
  • Commitment to ongoing professional development to ensure quality in the delivery of care.
  • Active driver's license and access to a car.
