We are seeking a motivated and enthusiastic Social Worker to join our Single Point of Access Team in North Somerset. As a key member of our team, you will be responsible for providing timely advice, information, and triage of referrals to adult care and community health services.
Key Responsibilities:
- Work collaboratively with a diverse team of Health and Social Care professionals to provide effective services to residents of North Somerset
- Develop and maintain responsibility for your own workload, effectively risk assess, and make timely decisions for complex situations
- Contribute to the development of our strengths-based, proportionate approach to assessment, enabling choice and independence for residents

Requirements:
- Registered Social Worker with a strong understanding of adult care and community health services
