Clinical Care Specialist

Position Purpose:
This position combines the roles of Clinical Care Specialist and Education Coordinator, providing the employee with the benefit of working across two key areas. The purpose of the position is to support the General Manager and Regional Care Assessment Consultants in creating and storing accurate ACFI appraisals. This will be achieved by providing leadership in the assessment of residents and delivering education initiatives to team members on optimal application of the ACFI.
About the Role:
- Permanent Part-time Opportunity: Work 3 days a week, Monday to Friday, office hours
- Current Practicing Registration: Must have a current practicing registration as a Nurse with AHPRA for at least 2 years
- Additional Certifications: Certifications in relation to ACFI or Quality activity are highly desirable
About You:
- ACFI Documentation and Reporting: Accurate ACFI documentation and monthly reporting skills
- ACFI Activities Management: Experience managing ACFI activities, including assessments, collating answer appraisals, and successful submission of claims to Medicare
- Resident ACFI Review Process: Ensure the resident ACFI review process operates efficiently and effectively
- Data Interpretation and Analysis: Demonstrated ability to interpret and analyse data
- ACFI Knowledge Improvement: Demonstrated improvement in ACFI knowledge by Nurse/Care team member at care community
- Training Delivery: Capacity and knowledge to deliver training to all newly team care members and at regular intervals to all current team members as identified by Manager
- Clinical Education Sessions: Comprehensive clinical skills and knowledge to provide one-on-one informal clinical education sessions where required
