About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments to identify individuals' needs and develop effective care plans.
  • Deliver evidence-based interventions, including brief psychosocial interventions, to promote recovery and well-being.
  • Work collaboratively with individuals, carers, and multidisciplinary staff to develop and implement risk management plans.
  • Provide short-term, low-level interventions to prevent escalation and crisis.
Person Specification
  • RMN/RNMH/Social Worker equivalent qualification.
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness.
  • Proven experience of working with clients with complex needs.
  • Experience of working in multi-disciplinary teams.
Desirable Skills and Qualifications
  • Completion of Preparation for Mentorship/Practice Teacher/Fieldwork Educator qualification or willingness to complete.
  • Primary care mental health work experience.
  • Experience of working with a psychological model of care.
  • Experience of triaging referrals.
