As a Support Practitioner, your role will involve working with individuals who have a wide range of support needs, providing them with practical and emotional support and encouraging them to achieve their own personal outcomes in all aspects of their daily lives. This may include keeping safe, meaningful activities, community involvement, physical health, relationships, emotional health and wellbeing.
You will be responsible for delivering person-centred support that is tailored to each individual's needs, goals and preferences. You will also contribute to the development and implementation of care plans, working closely with colleagues and external partners to ensure that individuals receive the best possible support.
