Job Overview

This is a newly formed team in Merthyr Tydfil County Borough Council that has been established to meet the requirements of the Discharge to Assess policy and to provide effective, seamless support to adults who are being discharged from hospital.

Key Responsibilities
  • Assess individuals in hospital or being discharged, identifying their support needs and developing a plan to ensure a safe transition back into the community.
  • Provide evidence of all qualifications specified as essential.
  • Ability to speak Welsh is desirable.

The successful candidate will be responsible for working with individuals, families, and healthcare professionals to assess and address the complex needs of adults being discharged from hospital. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Salary: Estimated £28,000 - £32,000 per annum, dependent on experience.
