About the Role

We are seeking a highly motivated and enthusiastic Physiotherapist to join our Home First services within Doncaster. As part of our integrated community care offer, we are developing our community physiotherapy service alongside partners to support clients in their own home.

Main Responsibilities
  • Provide specialist physiotherapy assessments over a range of conditions and complexities using advanced clinical reasoning to analyse clinical and non-clinical findings to form accurate physiotherapy diagnoses and prognoses.
  • Assess patient understanding of assessment and treatment in accordance with the Consent Policy, and assist members of the team to work within the legal framework for patients who lack capacity to consent to treatment.
  • Provide health education, health promotion, and prevention advice.
  • Formulate and recommend to others the best course of physiotherapy intervention and/or treatment programmes to meet the agreed goals set with the individual patient/or carer, using evidence-based practice and a wide range of treatment skills and options.
  • Use a range of verbal and non-verbal communication methods to communicate effectively with patients and carers to maximise rehabilitation potential.
  • Overcome barriers to appropriate rehabilitation, including communication difficulties, anxiety, or motivation.

Person Specification
  • Degree or Diploma in Physiotherapy.
  • Member of the Chartered Society of Physiotherapy (CSP).
