We are looking for a compassionate and caring individual to join our team as a Home Support Worker. As an Adult Care Assistant with City and County Healthcare Group Ltd, you will provide high-quality care and support to clients in their own homes.

Job Summary

This is a unique opportunity to make a real difference in people's lives by providing personalized care and support. You will work closely with our clients to understand their needs and preferences, and develop a tailored care plan that meets their requirements.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and other healthcare professionals. You will also be highly organized, with a keen eye for detail and the ability to prioritize tasks effectively.

Key Responsibilities
  • Providing personal care and support to clients in their own homes, including assistance with daily living activities such as bathing, dressing, and medication management.
  • Developing and implementing individualized care plans that meet the unique needs and preferences of each client.
  • Working collaboratively with other healthcare professionals, including nurses, therapists, and social workers, to ensure seamless care coordination.
  • Maintaining accurate records and documentation of client interactions, care plans, and progress.

Competitive pay rates: £12 per hour (weekdays), £12.50 per hour (weekends), plus 28p mileage. We offer a range of benefits to support your physical and mental well-being, including access to our Benefits and Well-being Platform and Cycle to Work scheme.
