About the Care Manager:

We seek an experienced, creative, and professional Care Manager to join our wonderful Fairmile Grange Care Home in Christchurch.

You will be responsible for overseeing the recreational, therapeutic, and social aspects of our activity programme, tailoring it to meet the needs, wishes, and desires of our residents, enabling them to live healthier, more independent, and fulfilling lives.

You will manage a small team of Wellbeing Assistants and work with them to create and deliver group and individualised activity plans based on our residents' interests, abilities, and life experiences.

Key Responsibilities:

  • Collaborate with the home manager and clinical lead to facilitate and support the completion of Biographies, promoting their use among colleagues to better understand our residents.
  • Liaise with residents, relatives, friends, and colleagues to gather comprehensive knowledge of each resident's preferences, dislikes, interests, abilities, and difficulties.
  • Utilize your skills to encourage conversation and serve as an educator within the home and community, promoting activity, wellbeing, and dignified living.
  • Contribute to keeping residents mobile and ensure safe moving and handling practices, following policies and procedures regarding falls management.
  • Develop daily programmes incorporating movement and exercise for groups and individuals at risk of falls.
  • Communicate events and opportunities through regular newsletters and other channels throughout the home.
  • Closely collaborate with the home manager and care teams to inform key colleagues about the Wellbeing team's activity schedules for residents.
  • Liaise daily with the senior person in charge and attend 10@10 meetings with Heads of Departments.
  • Explore effective methods and resources for meeting activity needs, including group events, individual sessions, internal, external, and community-based initiatives.
  • Establish relationships with local and national organizations to enhance the wellbeing offer at Fairmile Grange.
  • Maintain and develop existing programmes like our memory clinic, relative groups, Church services, which promote community involvement.

Requirements:

  • Experience working with vulnerable adults
  • Proven track record in a position of responsibility requiring initiative
  • Customer service experience
  • Excellent communication skills
  • Experience organizing activities programmes
  • Able to compile activity care plans
  • Strong interpersonal, verbal, and written skills
  • Able to maintain confidentiality

  • IT literate and able to utilize social media
  • Able to assess risk and construct appropriate risk assessments
