Band 5 Staff Nurse or Therapist

We are seeking a highly skilled and experienced Band 5 Staff Nurse or Therapist to join our Community Transfer of Care Hub team. As a key member of our multidisciplinary team, you will play a vital role in managing referrals into our Discharge to Assess Pathways across Dartford, Gravesend, and Swanley.
Key Responsibilities- Undertake patient care needs assessments to identify ongoing support required for safe acute hospital discharge.
- Visit patients in their homes to undertake face-to-face assessments of patient needs.
- Source appropriate care provision in the correct setting to meet these needs.
- Ensure sourced care provision is implemented from the point of hospital discharge.
- Communicate effectively with patients, carers, and all partner organizations.
- Maintain accurate patient records in line with professional and organizational requirements.
- Implement and work to ensure evidence-based care/best practice.
- Initiate health promotion activities with individuals, patients, and carers.
- Provide in-reach assistance to acute hospitals by providing expertise in community services to improve discharge and flow for pathways 1, 2, and 3.
- Undertake mentorship training, facilitate the teaching and learning experiences for all students and colleagues.
- Liaise with external agencies to expedite Transfer of Care Packages and Nursing Home/residential Home placements.
