About the Role

We are seeking a skilled and compassionate Relapse Prevention Specialist to join our team at Change Grow Live. As a key member of our service, you will play a vital role in supporting individuals struggling with substance misuse.

Key Responsibilities
  • Provide screening, assessment, and recovery planning to service users, ensuring a comprehensive understanding of their needs and goals.
  • Develop and deliver structured content within group settings, promoting recovery, resilience, and peer support.
  • Collaborate with co-facilitators to review progress, identify concerns, and tailor workshops to meet the needs of service users.
  • Engage with service users to support social reintegration, enabling them to lead meaningful and purposeful lives.
  • Contribute to the design and preparation of subject matter and materials for groups/pods, ensuring a high-quality service.
About You

We are looking for an individual with a strong understanding of substance misuse issues and experience working within a similar field. You will have excellent communication skills, both verbal and written, and be able to work effectively in a team environment.

  • Have a good working knowledge of mental health interventions, services, and good practice.
  • Be able to manage change successfully, prioritizing the needs of service users.
  • Understand the importance of information governance processes and commit to following necessary safeguards.
  • Be a supportive team player with strong interpersonal skills, able to work in partnership with a wide range of professionals and stakeholders.
