Job Description:

We are seeking a highly skilled District Nurse Specialist to join our team at Norfolk Community Health and Care NHS Trust. As a District Nurse, you will be responsible for providing high-quality care to patients in their own homes, working collaboratively with a multidisciplinary team to deliver safe and effective services.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans using clinical reasoning skills.
  • Prescribe and deliver care to support self-management and improve quality of life for patients in the community.
  • Collaborate with healthcare professionals, including GPs, social services, and other community stakeholders.
  • Maintain competencies and training in specialist skills, such as bowel care and venepuncture.
  • Liaise with patients, carers, and families to ensure accurate communication and cooperation.


Essential Qualifications and Experience:
  • Diploma or degree in nursing.
  • Current NMC registration.
  • Experience in a clinical setting.


Salary: £28,000 - £36,000 per annum (dependent on experience).
