About the Role

This is an exciting opportunity to join our team as a Support Practitioner, working directly with adults and supporting them with all aspects of their life.

As a Support Practitioner, you will be responsible for developing life skills such as communication, social skills, and encouraging independence.

You will work with individuals in their own homes, supporting them with personal care, medication administration, meal preparation, and household tasks.

You will also encourage involvement in activities such as social gatherings, outings, and hobbies, promoting a healthy lifestyle and independence.

Key Responsibilities
  • Support individuals with daily living tasks, such as bathing, toileting, and medication administration
  • Encourage independence and develop life skills, such as communication and social skills
  • Work with individuals to develop and implement personalized care plans
  • Provide emotional support and companionship to individuals
  • Collaborate with other professionals, such as healthcare workers and social workers, to ensure comprehensive support
