Youth Independence Coach

The Care Leavers Service Advisor will provide support to young people aged 17-24 to help them transition successfully to adulthood. The role involves developing and maintaining respectful, supportive relationships with young people, creating and implementing Pathway Plans in collaboration with young people, and keeping accurate case records and maintaining up-to-date documentation.

This is a dynamic role where no two days are the same, and you will have the opportunity to build meaningful relationships while contributing to positive outcomes.

• Provide support to young people to help them transition to adulthood.
• Develop and maintain respectful, supportive relationships with young people.
• Create and implement Pathway Plans in collaboration with young people.
• Keep accurate case records and maintain up-to-date documentation.

The successful candidate will be accountable for:

• Developing and maintaining relationships with young people and partner agencies.
