Job Title: Oncology Triage Specialist Practitioner

About the Role:

We are seeking an experienced registered nurse to join our Oncology and Haematology Triage Team as a Specialist Practitioner. The successful candidate will provide expert telephone assessment and initiation of treatment plans for patients contacting or attending the Cancer and Haematology Centre.

Key Responsibilities:

  • Provide expert physical and telephone assessment to patients with toxicities and symptoms caused by anti-cancer therapy or a consequence of their disease.
  • Be a defined point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice, and psychosocial support.
  • Work collaboratively and in partnership with other healthcare professionals, offering advice to colleagues.
  • Assess health and psychosocial needs of individual patients and provide care to meet these needs through a process of shared decision-making.
  • Accept referrals to agreed criteria and work in partnership with other professionals to deliver coordinated programs of care effectively and referring patients to other practitioners as required.

Requirements:

  • Registered nurse (part 1, 8, 12, or 15 of the register depending on the area of practice).
  • Postgraduate level study (HEI level 7) or demonstrable postgraduate level working evidenced via a continuous professional development portfolio.
  • First-level degree (HEI level 5 or 6) in nursing, midwifery, or equivalent.
  • Relevant specialist post-basic program to Master's Level or equivalent.
  • Teaching/Assessment/Mentoring qualification.

Desirable:

  • Masters (HEI level 7) degree.
  • Evidence of being able to work across organizational boundaries.
