Clinical Care Specialist

We are seeking a highly skilled and compassionate Clinical Care Specialist to join our team at Chestnut Tree House. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality clinical care to children with life-limiting conditions.
Our ideal candidate will have experience working in paediatric palliative care and will possess excellent communication skills, empathy, and a strong commitment to providing exceptional patient care.
**Key Responsibilities:**
- Develop and implement individualized care plans in collaboration with the multi-disciplinary team.
- Provide hands-on nursing care, including medication administration, wound management, and pain control.
- Participate in on-call rotations to ensure 24/7 coverage and provide emergency response support as needed.
