Transforming Adult Community Services

We are seeking a highly skilled and motivated District Nurse/Case holder to join our Integrated Care Teams at Croydon Health Services NHS Trust. As a key member of our team, you will provide expert, individualized patient-centered clinical care for clients with compromised skin integrity and pressure ulcers.

Main Responsibilities
  • To promote evidence-based practice for the population of Croydon Health Services (CHS) by assessing and planning care for patients referred to the service and working in conjunction with the tissue viability team.
  • To provide education and support to patients, carers, and other healthcare professionals on pressure ulcer prevention and management.
  • To work closely with other healthcare professionals to promote a high standard of care in the prevention and management of pressure ulcers, ensuring adherence to local and national guidelines and best practices.

Requirements
  • A minimum of 2 years nursing experience at Band 5 in the community
  • Experience as a mentor for student nurses
  • Experience of caseload management
  • Experience as a sign-off mentor for student nurses
  • Experience of financial and human resources issues
Skills and Qualifications
  • Ability to assess, plan, implement, and evaluate nursing care for patients in their own homes
  • Demonstrate a high level of clinical competence
  • Understand the change management process
  • Clear verbal communication skills with patients, carers, and staff at all levels
  • Ability to prioritize own and teams' workload and organize time effectively
Working Conditions

The ideal candidate must have a clean full UK driving license and access to a car for work purposes. The role requires flexibility, including unsocial hours and weekends, with a willingness to actively participate in team and service development activities.
