Clinical Assistant Practitioner

Reports To: Clinical Lead
Job Purpose:We are seeking a dedicated and compassionate Assistant Practitioner to join our client's multi-disciplinary community and acute-based service. This role involves working with adults with complex physical and neurological disabilities, providing crisis intervention to reduce hospital readmissions and facilitate early discharge from A&E and Frailty settings.
Key Responsibilities:- Contribute to intermediate care and unplanned services, ensuring seamless transitions for patients.
- Work independently with clients on treatment programmes developed under the supervision of qualified therapists and nurses, promoting autonomy and self-management.
- Assist in the assessment, treatment, and evaluation of clients' needs to avoid unnecessary hospital admissions and ensure swift discharge, reducing healthcare costs and improving patient outcomes.
- Implement treatment programmes and provide necessary equipment, ensuring clients receive optimal care and support.
- Conduct initial assessments of patients referred following a fall and report to senior clinicians, identifying potential risks and developing strategies to mitigate them.
- Maintain communication with therapists and nurses, reporting any changes in patients' conditions, and collaborating to develop comprehensive care plans.
- Achieve competencies in initial assessments and work with Occupational Therapy, Nursing, and Physiotherapy to deliver individual treatment programmes, promoting interdisciplinary collaboration and best practice.
- Support the smooth running of the ITOC through a flexible and adaptable approach to work, including administrative tasks, and contributing to a positive and productive team environment.
- Achieve advanced competencies for falls assessments and multi-disciplinary working, ensuring a high level of expertise and confidence in clinical practice.
- Assist in the day-to-day delivery of the ITOC team services, including hospital discharge, A&E, Frailty, and Discharge to Assess, promoting efficient and effective care pathways.
- Participate in team meetings and develop an understanding of the roles within the team, fostering collaboration and communication.
- Assist in moving and handling clients using specialist equipment, ensuring safe and dignified care.
- Support clients and carers by implementing individual therapy programmes and providing information for treatment evaluation, promoting empowerment and self-advocacy.
- Manage challenging behaviour and report relevant information to qualified staff and agencies, ensuring a safe and respectful environment for all.
- Experience working in a community setting, with a strong understanding of the needs and challenges faced by adults with complex physical and neurological disabilities.
- Ability to work autonomously, with a high level of self-motivation and initiative, and a willingness to take on new challenges and responsibilities.
- A valid driving license and access to a car, with a willingness to travel to different locations as required.
