Job Title: Adult and Community Skills Coach

We are seeking a highly motivated and experienced nurse to join our Discharge Team as a Band 6 Discharge Specialist Nurse. The successful candidate will be responsible for leading a team of discharge coordinators and providing clinical leadership for discharge planning.

Key Responsibilities:
  • Lead a team of discharge coordinators to ensure timely and safe discharges for patients.
  • Provide clinical leadership for discharge planning, including assessing patient needs and developing care plans.
  • Work closely with the multidisciplinary team to identify same-day and future discharges, and to address any delays in service.
  • Attend daily patient flow meetings to provide discharge numbers and escalations to support early discharges.
  • Develop and maintain relationships with community services to ensure seamless provision of care delivery.
Requirements:
  • Registered Nurse, Physiotherapist, or Occupational Therapist with significant post-registration experience at Band 5 or equivalent.
  • Excellent written and verbal communication skills, with the ability to act professionally at all times.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
