About the Job: As a Community Support Worker with Pennine Care NHS Foundation Trust, you will play a vital role in supporting adults with mental health needs to live independently in their own homes or community settings. This is a challenging yet rewarding role that requires strong communication and interpersonal skills, as well as the ability to work collaboratively with other healthcare professionals.

The Ideal Candidate:
We are looking for a highly motivated and compassionate individual who is able to build positive relationships with service users and their families. You will have a good understanding of mental health issues and how they impact on individuals, as well as excellent written and verbal communication skills. Basic IT skills and a NVQ 3 or equivalent in Health and Social Care are also essential requirements for this role.

Responsibilities:
  • Develop and maintain effective relationships with service users and their families
  • Conduct assessments to identify service users' needs and goals
  • Work with other healthcare professionals to develop and implement individualised care plans
  • Provide emotional support and practical assistance to service users
  • Maintain accurate and confidential records

Working Hours: 37 hours per week, Monday to Friday. Some flexibility may be required to meet the needs of service users.

Salary: £25,000-£30,000 per annum dependent on experience.
