About the Role:
As a Social Worker in our Older Persons Team, you will be responsible for providing high-quality care and support to vulnerable adults. You will work closely with our clients, their families, and other healthcare professionals to ensure that our clients receive the best possible care.
Key Responsibilities:
- Conduct assessments and develop care plans to meet the needs of our clients
- Provide emotional support and guidance to our clients and their families
- Work collaboratively with other healthcare professionals to ensure seamless care delivery
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
To be successful in this role, you will need:
- A degree in Social Work or a related field
