Job Summary:
We are seeking a highly skilled Registered Mental Health Nurse to join our multi-disciplinary memory assessment service team. As an independent specialist practitioner, you will be responsible for ensuring the effective assessment, planning, consultation, and formulation towards diagnosis.
Key Responsibilities:
• Develop and maintain a specialist knowledge and understanding of strategies and interventions in the treatment of dementia.
• Refer to other agencies within the service or to other services or agencies where necessary.
• Identify risk factors and take appropriate action.
• Provide a communication structure between GPs and other services in LCFT, non-statutory services, to ensure the highest possible standards of patient care.
• Mentor and provide clinical supervision to other staff and trainees on placement.
• Promote the Trust values within the MAS service and in accordance with the NMC and HPC code of conduct.
• Assist the team manager in ensuring that leadership within the MAS team provides effective assessment, formulation, planning, and monitoring of care given to patients and their carers.
