Key Responsibilities
The Clinical Nurse Specialist will be responsible for:
- Developing, implementing, and assessing personalized treatment plans for both acute and chronic conditions.
- Assisting the clinical team in formulating treatment strategies for patients with chronic diseases.
- Delivering routine nursing care in alignment with clinical guidelines and best practices.
- Prescribing medications within the scope of practice and established protocols.
- Supporting patients in adhering to prescribed medication regimens.
- Providing specialized wound care and travel medicine services.
- Coordinating pathology services and interpreting results as necessary.
- Referring patients to appropriate services when required.
- Conducting chronic disease management clinics and facilitating referrals to secondary care as needed.
- Maintaining accurate clinical documentation in compliance with current legislation.
- Utilizing SNOMED codes effectively for clinical record-keeping.
- Managing chronic disease registers and ensuring comprehensive patient follow-up.
- Assisting with minor surgical procedures under the guidance of GPs.
- Implementing vaccination programs for various age groups.
- Collaborating with external agencies to provide holistic support to vulnerable patients.
