Job Role

We are seeking a dedicated Community Care Coordinator to join our team at CLCH NHS Trust. As a key member of our community health services, you will play a critical role in coordinating health promotion initiatives and ensuring seamless service delivery.

Main Job Functions:

  1. To provide a skilled, effective, and evidence-based health visiting service to clients in various community settings.
  2. To apply specialist skills and knowledge in delivering the health visiting service.
  3. To consult, advise, and educate individuals and families on good healthcare practices.
  4. To develop health profiles of the defined population and local communities, identifying health needs of disadvantaged groups and targeting outreach services.
  5. To identify, prioritise, and implement programmes of care that meet public health needs.
  6. To provide parenting skills support and information through coordinating/participating in parenting groups and one-to-one home visits.
  7. To provide health assessments, advice, and education to families and individuals of all ages.
  8. To develop and implement regular health promotion initiatives on specific health issues.

Desirable Qualifications and Experience:

  • Registered Nurse or Registered Health Visitor
  • Current NMC registration
  • Evidence of continuing post-registration education and training
  • Recent health visiting experience
  • Mentor training or willingness to work towards it

Annual Salary Range: £45,000 - £55,000
