Main Duties- Provide robust assessments of patients who have been referred due to presenting with mental health difficulties.
- Identify appropriate care pathways and ensure the least restrictive option to meet the needs of the patient and ensure their well-being.
- Signpost to appropriate care providers, including primary and tertiary care.
- Raise concerns to senior staff for support and management.
- Complete documentation and planning care for patients.
- Present cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaise with colleagues, including nursing, occupational therapy, medics, psychology, and admin.
This role will involve working with patients who have been referred due to presenting with mental health difficulties. You will be responsible for providing robust assessments, identifying appropriate care pathways, and ensuring the least restrictive option to meet the needs of the patient and ensure their well-being. You will also be required to signpost to appropriate care providers, raise concerns to senior staff, and complete documentation and planning care for patients.
Requirements- A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices.
- A full UK driving licence and own vehicle.
