About the Role

We are seeking a skilled and compassionate Community Nurse to join our Surrey Heath Community Nursing Team. As a Community Nurse, you will play a vital role in delivering high-quality care to patients in their own homes.

Key Responsibilities
  • Conduct holistic assessments, develop care plans, and deliver evidence-based care to patients in their own homes.
  • Work collaboratively with the multidisciplinary team to ensure seamless care delivery.
  • Implement and maintain high standards of care delivery, adhering to organizational policies and procedures.
  • Communicate effectively with patients, carers, and healthcare professionals to ensure comprehensive care.
  • Contribute to the development and implementation of innovative care solutions to improve patient outcomes.
