DigiKape is seeking a highly skilled and experienced Social Worker to join our Adults Community/Locality team in Knowsley. As a key member of our team, you will be responsible for delivering high-quality social care services to adults in the area.
Key Responsibilities:
- Conduct comprehensive social care needs assessments to identify individual requirements and develop personalized support plans.
- Collaborate with service users, families, and other stakeholders to ensure effective risk management and safeguarding.
- Develop and implement commissioning strategies to secure necessary resources and services.
- Participate in risk assessments and manage risks using the MARAMS framework.
- Ensure compliance with relevant legislation, including the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DOLS), and the Care Act.
- Work closely with other professionals to deliver high-quality care and support services.
Requirements:
- Proven experience as a Social Worker in a similar role.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong analytical and problem-solving skills.
- Valid driving license and access to a vehicle.
Working Arrangements:
This is a full-time position, working 36 hours per week, with a hybrid working pattern that includes mandatory office days on Wednesday and Friday, and other training and duty days as required.
