**About the Role**

We are seeking a highly skilled and experienced Integrated Discharge Team Practitioner to join our team at Lancashire & South Cumbria NHS Foundation Trust.

The successful candidate will be responsible for providing clinical management and coordination of complex mental health discharge pathways, working collaboratively with a wider multi-organisational team as part of co-produced service developments.

As an Integrated Discharge Team Practitioner, you will use your experience and knowledge of specialist discharge pathways and out-of-hospital provision to influence discharge decisions, promoting alternatives to restricted environments and working with patients and clinical teams to increase confidence in combined impact of multi-agency discharge support.

The post holder will require a working knowledge of commissioning processes and have several years of post-qualifying experience.

**Key Responsibilities**

• Use expertise in risk management, patient safety, and prioritisation to inform discharge decisions.

• Work closely with ward Multi-Disciplinary Teams to ensure effective discharge planning.

• Contribute to and review discharge plans to ensure patients are progressing to the least restrictive environment that meets their needs for safety and service quality.

• Make recommendations for patients suitable for transfer to less restrictive environments and support Care Coordinators with this process.
