Care Coordinator Specialist

**Job Title:** Assessment Support Coordinator
We are seeking a dedicated and compassionate individual to join our East Dorset Locality Team as an Assessment Support Coordinator. This full-time role is based in Ferndown, where you will work closely with Social Workers, Occupational Therapists, and health colleagues to support individuals in achieving their independence goals.
The successful candidate will undertake strengths-based assessments and support planning in accordance with the Care Act 2014. You will assist individuals in identifying their goals and developing strategies to achieve them, including completing mental capacity assessments when necessary.
This role requires excellent communication skills, a flexible approach, and the ability to work effectively in a fast-paced environment. Previous experience in social care is desirable, but applications from candidates with transferable skills are also welcome.
In return for your hard work and dedication, we offer a competitive salary of £30,000 - £40,000 per annum, depending on experience. Our benefits package includes hybrid working, regular supervision, access to mentoring and coaching, and a comprehensive induction plan.
