We are seeking a highly skilled and experienced Practice Pharmacist to join our team at DHU Healthcare. As a Practice Pharmacist, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.

About the Role

This is an exciting opportunity for a motivated and dedicated pharmacist to work in a dynamic and supportive environment. As a Practice Pharmacist, you will be responsible for:

  • Providing clinical expertise and guidance to patients and healthcare professionals
  • Conducting clinical medication reviews and audits to improve patient outcomes
  • Developing and implementing medicines optimisation strategies to enhance patient care
  • Liaising with community and hospital pharmacy teams to ensure seamless care transitions
  • Working collaboratively with the multidisciplinary team to achieve collective brilliance

Requirements

To be successful in this role, you will need:

  • Completion of an undergraduate degree in Pharmacy
  • Registration with the General Pharmaceutical Council (GPhC)
  • Basic Life Support and Child and Adult Safeguarding certification
  • At least 2 years' experience as a pharmacist, demonstrated in a practice portfolio
  • Experience of clinical medication reviews, clinical audits of prescribing, and using SystmOne
