We are seeking an experienced Health and Social Care Professional to join our team as a Case Manager - Catastrophic Injury Specialist. As a key member of our team, you will be responsible for coordinating care and rehabilitation for young people and adults with catastrophic injuries.
Key Responsibilities
- Coordinate care and rehabilitation plans tailored to the specific needs of clients
- Work closely with clients, their families, and care teams to support physical, cognitive, and emotional wellbeing
- Manage a client caseload and conduct home visits within a designated geographical area
- Participate in ongoing mentorship, training, and professional development opportunities
