Community Nurse

We are seeking an experienced and self-motivated nurse to join our Urgent Care Nursing service in Plymouth, South Hams, and West Devon. As a key member of our team, you will provide high-quality nursing care to individuals in their homes, supporting them to remain safe, warm, and independent.
The successful candidate will have a strong background in nursing and health care, with a focus on acute care in the community. You will be responsible for assessing, planning, implementing, and evaluating care for patients, working autonomously and unsupervised in the community setting.
Key Responsibilities- Provide holistic evidence-based care to patients in accordance with national and organisational policies and procedures.
- Assess, plan, implement, and evaluate care on a day-to-day basis.
- Work autonomously and unsupervised in the community setting, recognising the risks associated with lone working.
- Manage and prioritise delegated workload according to local guidelines and identified need.
- Support other members of the team in their planning of patient care.
- Review care delivery and maintain records in accordance with local and NMC guidelines.
