Patient Care Coordinator

**Job Summary:**
We are seeking a skilled and compassionate Patient Care Coordinator to join our team in Guildford, Surrey.
As a Patient Care Coordinator, you will play a critical role in ensuring the safe and efficient transportation of vulnerable individuals with mental health conditions. Your primary responsibilities will include:
- Coordinating the transportation of service users between various locations including hospitals, homes, and prisons.
- Liaising with hospital staff and the police on the safe conveyance of service users to and from places of safety.
- Ensuring that the ambulance is kept safe, clean, and tidy.
You will also be responsible for assisting service users with challenging behavior and/or mental health illnesses, and transferring them safely between vehicles and locations.
