Key Responsibilities:

  • Work collaboratively with multidisciplinary teams to develop and implement individualized care plans.
  • Conduct assessments and evaluations to identify patients' needs and goals.
  • Develop and implement treatment plans to address patients' needs and goals.
  • Provide education and support to patients and their families to promote recovery and well-being.

Requirements:
