About the Role

The Health and Wellbeing Practitioner will deliver client-centred treatment, managing a caseload and providing harm reduction advice and guidance. This role involves assessing, planning, and delivering structured 1:1 and group-work sessions for Substance Misuse and Low-Level Mental Health service users.

Key Responsibilities

  • Manage a caseload of clients requiring substance misuse and low-level mental health support
  • Provide harm reduction advice and guidance to clients
  • Assess, plan, and deliver structured 1:1 and group-work sessions
  • Work collaboratively with other Health and Wellbeing agencies to provide transitional support from prison to the community

Requirements

  • Consecutive 3-year residency in the UK and right to work in the UK
