We are seeking a skilled and dedicated Community Nurse to join our team at Oxford Health NHS Trust. As a Community Nurse, you will play a vital role in delivering compassionate care to our community members, providing increasingly complex care for patients and support for family members.
Key Responsibilities- Provide high-quality patient care in a community setting
- Work collaboratively with other healthcare professionals to deliver effective care
- Develop and maintain strong relationships with patients and their families
- Contribute to the development of care plans and implement evidence-based practice
- Participate in quality improvement initiatives to enhance patient outcomes
- Registered General Nurse (adult) qualification
- Education to degree level or equivalent
- Knowledge of long-term conditions and their management
- Evidence of ongoing professional development
- Previous community services experience (desirable)
