The successful candidate will work closely with a multidisciplinary team, delivering holistic care and supporting patients with a variety of complex needs, including chronic conditions, palliative care, wound management, and rehabilitation.
Key Responsibilities:
- Provide high-quality nursing care to individuals in the community, ensuring the needs of patients are assessed and met.
- Lead and manage a small team of community healthcare professionals.
- Work collaboratively with GPs, social care, and other allied health professionals to deliver integrated care.
- Support patients and their families with education and self-management strategies.
- Promote health and wellbeing through proactive intervention and health education.
- Maintain accurate and up-to-date patient records.
Essential Requirements:
- Registered Nurse (Adult) with valid NMC registration.
- District Nurse qualification (Specialist Practitioner Qualification) or equivalent experience.
- Experience in community nursing and delivering care to patients with complex needs.
- Strong leadership skills and the ability to manage and support a team.
- Excellent communication and interpersonal skills.
- Full UK driving licence and access to a car for work purposes.
