About the Role

We are seeking a highly motivated and experienced Health and Social Care Support Navigator to join our team at AWD online. As a Health and Social Care Support Navigator, you will play a vital role in supporting vulnerable adults in prison and community settings.

Key Responsibilities
  • Provide Specialist Health Care Navigation Services to individuals being released from prison, supporting people both pre and post release in line with service guidelines.
  • Work alongside other agencies to improve health outcomes and reduce reoffending.
  • Complete an initial screening and care mapping assessment with each service user and use service documents to plan their care and release.
  • Develop tailored models of support for patients through a variety of modalities, mediums, and with varying intensity to promote independent self-management and engagement with statutory and non-statutory services.
  • Support health promotion, planning, and delivering health promotion events in collaboration with colleagues.
  • Complete detailed, accurate, professional, and timely notes for each patient.
  • Provide a coaching framework to empower individuals with independent management of their release and associated healthcare needs.
Requirements
  • Educated to NVQ Level 3 or equivalent with qualifications or training relevant to the role, e.g., Health and Social Care/Mental Health.
  • Relevant experience of supporting vulnerable individuals to identify, set, and meet their own goals using 1:1 and/or group interventions.
  • Working knowledge of some of the following: health, debts, social care, housing, court systems, mental health, criminal justice, and substance misuse.
  • Experience of working to support individuals using structured, time-limited support.
  • Passionate and motivated to support individuals leaving the criminal justice system through provision of a high-quality, professional level of care.
  • Patient, calm, and respectful.
  • Hold a full UK driving license and have access to own transport for work purposes.
