Mental Health Transition Coordinator

Coleg Cambria is seeking an experienced and compassionate Mental Health Wellbeing Transition Officer to join our team. In this critical role, you will work closely with learners, teachers, and families to provide personalized support and guidance throughout their educational journey. With a focus on seamless transitions and holistic care, you will help create a supportive environment where young people can thrive.
About the Position:
As a key member of our team, you will:
* Provide targeted support to learners with mental health concerns
* Foster strong relationships with local schools and community organizations
* Collaborate with colleagues to ensure comprehensive support services
* Monitor learner progress and outcomes, reporting back to stakeholders
Essential Requirements:
* Relevant qualifications in mental health or wellbeing (Level 3 or above)
* Proven experience working with young people who have mental health concerns
* Exceptional communication and interpersonal skills
* Ability to maintain confidentiality and handle sensitive information
* Personal resilience to navigate challenging situations
* Digital literacy skills and ability to develop teaching methods
* Desireable: Welsh language proficiency
