**Job Overview**

We are seeking a skilled Occupational Therapist to join our team at St George's University Hospitals NHS Foundation Trust. This exciting opportunity is focused on providing innovative care for homeless individuals admitted to our hospital.

The successful candidate will provide clinical expertise and case management for patients referred to the service, taking into consideration the complex tri-morbid factors that this patient cohort experiences.

This role offers significant partnership working opportunities aimed at improving hospital discharge for individuals and groups of patients. You will work closely with NHS services, GP's, Housing departments, Social Care, and Voluntary Sector organisations to ensure holistic assessment, planning, and provision of interventions.

Main Responsibilities:

  • To provide direct support to the Homeless Team, medical and surgical teams, therapists, and wards in discharging patients safely and timely.
  • To work alongside a Specialist General Practitioner, a Homelessness Nurse Specialist, 2 Housing Officers, an Adult Violence Practitioner, and a Care Navigator to provide additional support to homeless individuals.
  • To contribute to the enhanced management of homeless people on the wards.

Essential Requirements:

  • Diploma/Degree in Occupational Therapy.
  • Registered with the Health Professionals Council as an Occupational Therapist.
  • Post-Graduate Training.
  • Teaching and Assessing course or mentorship course.

Estimated Salary: £38,890 - £44,414 per annum.
