Job Summary

We are seeking a skilled and compassionate Social Care Assessor to join our team in the Newton Abbot Intermediate Care Team.

About the Role

This is an exciting opportunity for a part-time Social Care Assessor to work in a busy community setting, promoting a person-centred and partnership approach to ensure the best possible outcomes for adults and their carers.

The successful candidate will be responsible for completing care and support needs assessments, developing and reviewing personalised care and support arrangements, and supporting people to promote their independence.

As a member of the integrated Health and Social Care team, you will work closely with other professionals to provide advice and information, make onward referrals, and support the team in a variety of contacts and referrals.

Key Responsibilities
  • Complete care and support needs assessments and develop and review personalised care and support arrangements
  • Support people to promote their independence and apply a strengths-based approach
  • Complete mental capacity assessments and raise and triage Safeguarding Concerns
  • Provide advice and information, work in partnership with other agencies, and make onward referrals
Requirements

We are looking for applicants who are confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a health or social care environment and have a good understanding of human needs.

You will also have experience of undertaking assessments and working with an eligibility criteria. The successful candidate will be jointly supported by the Intermediate Care and Therapy Lead and the Team Manager of the Newton Abbot Community Health and Social Care Team.
