About the Role

We are seeking a qualified and experienced Social Worker to join our Child Health and Disability (CHAD) team in Cardiff. As a key member of our team, you will work collaboratively with colleagues across various agencies and council directorates to provide high-quality social work services to children and young people with physical and learning needs.

Key Responsibilities
  • Undertake assessments and implement and review care and support plans, underpinned by the Social Services and Well-being (Wales) Act 2014.
  • Work alongside a multi-agency team of professionals, in partnership with families, to provide specialist advice and support.
  • Contribute to the development and maintenance of effective systems to ensure quality services.
  • Complete wellbeing assessments, child protection enquiries, and develop and review care and support plans.
  • Participate in regular team meetings and demonstrate a high level of good quality written and verbal practice.
Requirements
  • A Degree or Masters in Social Work Diploma in SW (or equivalent).
  • Experience of working within a Child Health & Disability Team.
  • Experience of working as part of a multi-disciplinary team around children.
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS.
  • 3 Years of referencing.
  • Driving license with access to a car.
