Job Title: Adult Mental Health Practitioner

We are seeking a highly skilled and experienced Adult Mental Health Practitioner to join our team at South West London and St Georges Mental Health NHS Trust.

About the Role

The successful candidate will be responsible for providing high-quality clinical care to individuals with mental health needs, working closely with our multidisciplinary team to ensure seamless service delivery.

Key responsibilities will include:

  1. Conducting comprehensive clinical assessments and developing individualized care plans.
  2. Providing expert advice and guidance to colleagues and stakeholders on mental health matters.
  3. Collaborating with other services to ensure effective service user pathways and outcomes.
  4. Maintaining accurate and up-to-date records of service user interactions and care plans.
  5. Participating in quality improvement initiatives and contributing to the development of clinical policies and procedures.
Requirements

To be successful in this role, you will need:

  • A degree in a relevant field (e.g., nursing, psychology, social work).
  • Significant experience in adult mental health services, preferably in a forensic or community setting.
  • Excellent clinical skills, including assessment, diagnosis, and treatment planning.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work autonomously and as part of a team, with a flexible approach to workload and priorities.
